- Your baby’s full name, if they have been born
- Your date of birth and your phone number and email
- Your insurance company
- Your baby’s date of birth, or your due date if you are pregnant
- A short note on what you would like help with
Insurance we work with
Most plans cover lactation support at no cost to you under the ACA. We bill your insurance directly and confirm your benefits before your visit.
If we don’t take your insurance
Our mission is care for every family. If you do not have one of the plans we work with, reach out anyway.
We can still see families without accepted insurance at no cost.
What happens after you reach out
This form is a request, not instant booking. A team member follows up to confirm your time, visit type, and benefits.
We aim to respond quickly, and we prioritize newborns and urgent feeding problems.
